Insurance Information
While we participate in most insurance plans, there are a few in which we do not. Please call your insurance company to confirm we are participating with your plan. As a courtesy, we will automatically file claims to all policies we participate with. If we do not participate in your plan and you still want to bring your child(ren) here to the office, payment will be expected at time of service and you will be given the proper documentation so you may file a claim on your behalf. Please be aware we do NOT accept First Health, Multiplan, Mega Life, Midwest Life, or Three Rivers; however, when you call they will tell you we are in network.
Please be sure to bring your insurance card with you to all appointments!
Fees and Payment
Co-pays, co-insurance, and deductibles are expected at time of service. This is a requirement of your insurance.
Every effort is made to ensure accurate billing; therefore, if you have a billing concern or any questions, please feel free to call our office.
Billing Office Hours
Our billing office is open Monday through Friday from 8:00 am to 4:30 pm. Please do not hesitate to call with any questions. Most questions can be answered by our billing office without having to call your insurance.
Other Information
Please be aware of your insurance benefits prior to your visit. Our office offers services, procedures, and vaccines that may/may not be covered by your plan. There is no way for us to know benefits for each patient due to the various plans each carrier offers.
